How to use Social Security Numbers for Genealogical Purposes

The Freedom of Information Act has made available copies of the original applications of those Social Security card holders who have passed away. Researchers must send $7.00, if the SS number is known, or $16.50, if the number is unknown or incorrect, to the following address:

Social Security Administration, Freedom of Information Officer, Office of Central Operations - Genealogy, 300 N. Greene Street, Baltimore, Maryland, U.S.A., 21235.

"I would like to obtain, under the Freedom of Information Act, and for genealogical purposes only, a microprint of the original Social Security Application Form (SS-5) for Mr. John Doe, SSN 123-45-6789."

The SS-5 form will include an applicant's full name, address, age, birthday, birthplace, parents (including mother's maiden name), sex, race, and in some cases employment information.

N.B. For people that have social security numbers between 700-728, the address is Railroad Retirement Board, 844 Ruch Street, Chicago, Illinois, U.S.A., 60611.

Issuing of Social Security cards began as early as 1937. The original applications for a Social Security Number (SS-5) contain name, address, occupation, plus, usually, the names of person's parents, including mother's maiden name.
